چکیده

Background and purpose: A likely explanation for the increase in lens fluorescence with aging and the acceleration of this process in patients with diabetes, is that lens proteins are altered by non-enzymatic glycosylation and the further steps of the Maillard reaction. Thus, the lens fluorescence intensity appears to represent a cumulative index of glycemia over the entire lifetime of the subject. Together with th: gradual onset of hyperglycemia in non-insulin dependent diabetes (NIDDM), which usually precedes the clinical detection of the disease by several years, this implies that patients with newly detected NIDDM should have increased lens fluorescence. We have investigated this theory on 14 patients with newly detected NIDDM and 11 age-matched healthy subjects with normal glucose tolerance. Results: Lens fluoresceme in 11 of tit: I4 diabetic patients where higher than the age-related mean plus 2 standard deviations of the healthy subjects. Within both groups lens fluorescence was significantly correlated with HbA,,, fasting plasma glucose and the two-hour glucose tolerance test levels. Conclusion: The results confirm that lens fluorometry may valuable as a population screening for the presence of NIDDM, although a prospective population study is needed to determine how effectively this can be done.

Long-term remission of myopic choroidal neovascular membrane after treatment with ranibizumab: a case report

INTRODUCTION Myopia has become a big public health problem in certain parts of the world. Sight-threatening complications like choroidal neovascularisation membranes occur in up to 10% of pathological myopia, and natural history studies show a trend towards progressive visual loss. There are long-term financial and quality-of-life implications in this group of patients, and treatment strategies...

Patchy atrophy and lacquer cracks predispose to the development of choroidal neovascularisation in pathological myopia.

AIMS To determine the incidence and predisposing findings for choroidal neovascularisation (CNV) in a large series of highly myopic patients. METHODS The medical records of 218 consecutive patients (325 eyes) with myopic fundus changes in the macula were reviewed. The incidence of CNV during a follow up of at least 3 years of highly myopic patients and identification of predisposing findings ...

Indocyanine green angiography in chronic central serous chorioretinopathy

BACKGROUND Central serous chorioretinopathy (CSC) is a condition that originates from alterations of the choroidal circulation. The aim of this paper was to evaluate the use of indocyanine green angiography (ICGA) in patients with chronic CSC. MATERIAL/METHODS The analysis included 17 patients (34 eyes) with chronic CSC in at least 1 eye. The eye examination included: distance and near visual...

Bruch's membrane and choroidal macrophages in early and advanced age-related macular degeneration.

AIM To determine the sub-macular Bruch's membrane (BrM) macrophage count and the choroidal and BrM macrophage immunophenotype in normal eyes and in eyes with early and advanced age-related macular degeneration (AMD). METHODS BrM macrophages were counted in 125 human eyes (normal, normal aged, early AMD and geographical atrophy), and CD68 and inducible nitric oxide synthase (iNOS) immunohistoc...

Outcome of iatrogenic choroidal neovascularisation in sickle cell disease.

Choroidal neovascularisation developed in 62/66 (94%) eyes followed up for a mean period of 11.2 years after treatment with xenon arc feeder vessel coagulation for proliferative sickle retinopathy (PSR). In 27 eyes the neovascularisation was chorioretinal alone and in 35 eyes choriovitreal. Chorioretinal neovascularisation (CRN) was a benign complication but may convert to choriovitreal neovasc...
